9. In order for a person to be admitted for the first time to the educational services offered by a school service centre, an application must be submitted to the school service centre under whose jurisdiction that person falls.
An application for admission must include the following information:
(1)  the person’s name;
(2)  the person’s residential address;
(3)  the names of the person’s parents, unless the person is of full age;
(4)  (subparagraph revoked).
